Creatine is the most-researched, most-reliable performance supplement money can buy — and one of the few where the science outruns the marketing. It's a compound your body already makes and stores in muscle to power short, hard efforts. Supplementing pushes those stores toward their ceiling, and the payoff — measurable gains in strength and power when you actually train — is about as well established as sports nutrition gets.1

What creatine actually is
Creatine is a small molecule your body builds from amino acids in the liver, kidneys, and pancreas — roughly a gram a day — and tops up with another gram or two from food. Red meat and fish are the main dietary sources, so people who eat little of either tend to carry lower muscle stores to begin with, a detail that matters a lot for who responds. About 95% of it is stored in skeletal muscle, most of that as phosphocreatine.14
Phosphocreatine is a fast-access battery. Your muscles run on ATP, and during a heavy set or a sprint they burn through it in seconds. Phosphocreatine donates a phosphate to regenerate ATP almost instantly, which is what lets you grind out one or two more hard reps before the tank runs dry. The more phosphocreatine you have banked, the faster you replenish that energy — and over weeks of training, those extra reps and that extra power compound into strength and size.1
Here's the catch that makes supplementing worthwhile: most people walk around with muscle creatine stores below their ceiling. What the evidence says, benefit by benefit
The grades above aren't all A's, and that's deliberate. A per-benefit grade is only useful if a weak case is shown as plainly as a strong one. Here's what earns each letter.
Strength and power output — Grade A
This is the claim creatine has earned many times over. It's the single most-studied ergogenic aid in sports science, and the pattern is remarkably consistent: pair creatine with resistance training and you get larger gains in maximal strength and power output than training alone, across efforts lasting under a few minutes.1 A systematic review and meta-analysis of lower-limb strength found a clear benefit that held regardless of age, sex, training status, or dosing protocol.6
The mechanism is the boring, believable one: creatine doesn't build muscle directly, it lets you do more quality work. A few extra reps per set, a bit more speed on the bar, more total training volume week over week — that added stimulus is what drives the strength adaptation. It's an A because it's been replicated in hundreds of trials and the effect shows up in real, trainable people, not just on a lab bench. In practice it tends to look like a few extra percentage points on your strength numbers over a training block — modest in any single session, but it compounds, and it's one of the most dependable edges you can get without a prescription.
Lean muscle mass — Grade B
Creatine reliably amplifies the muscle you build from training, which is a different and more modest claim than "creatine builds muscle." A meta-analysis in older adults found that adding creatine to resistance training produced about 1.37 kg more lean tissue mass than resistance training with a placebo.7 That's a real, useful bump — but it's an amplifier effect. Without the training stimulus, the muscle benefit largely disappears.
Two honest caveats keep this at a B rather than an A. First, some of the early scale increase is water pulled into muscle, not contractile tissue (the creatine side effects guide separates the water weight from the fat myth in detail). Longer-term lean gains are genuine muscle, but you have to look past the first couple of weeks to see them cleanly. Second, the effect size is meaningful but not dramatic, and it depends entirely on the training doing the heavy lifting. Strong evidence, contextual payoff.
Where this benefit earns its keep most is later in life. In older adults, holding onto lean mass and strength is tied directly to staying mobile and independent, and the resistance-training-plus-creatine combination is one of the better-studied levers for it — which is a big part of why creatine keeps surfacing in healthy-aging conversations, not just gym ones.7
Cognition — Grade C
This is where the story gets interesting and the certainty drops. Because the brain uses phosphocreatine to buffer its own energy demands, the theory is that extra creatine could help when the brain is energetically stressed. A systematic review of randomized trials in healthy people found some signal for short-term memory and reasoning — strongest in older adults and people with low baseline stores, and inconsistent in rested young adults.8 A 2024 study went further, showing that a single high dose improved cognitive performance and measurably shifted brain high-energy phosphates during sleep deprivation.9
That's genuinely promising, and it's why creatine keeps showing up in brain-health conversations. But "promising" is not "proven." The doses that move cognition are often higher than the muscle dose, the effects are context-dependent, and the trials are still small and mixed. The same low-baseline pattern from the muscle research seems to apply here too — vegetarians and stressed or sleep-deprived brains have the most to gain, while a well-rested omnivore may notice nothing. A C — limited human evidence, pointing in an encouraging direction — is the honest grade.
Bone health, with resistance training, in older adults — Grade C
Another early, training-dependent frontier. In a 12-month trial, postmenopausal women who combined creatine with a supervised resistance-training program preserved bone at the femoral neck better than those doing the same training with a placebo.10 That's a meaningful result in a population that badly needs options for bone.
The reasons it stays at a C: not every bone outcome in that trial separated from placebo, the literature is still just a handful of trials, and — importantly — creatine appears to do nothing for bone without the resistance training driving the adaptation. It's a "creatine plus training in older adults" signal, not a standalone bone supplement. The plausible mechanism is indirect — stronger muscles pulling harder on bone, plus creatine's role in the energy-hungry cells that remodel it — but plausibility isn't proof, and the trials haven't consistently delivered yet. Encouraging, not established.
Depression, as an add-on to antidepressant therapy — Grade D
This is the earliest, thinnest evidence on the page, and the grade reflects it. In one randomized, placebo-controlled trial, women with major depressive disorder who added 5 g/day of creatine to an SSRI showed greater improvement on a standard depression rating scale than those who added a placebo, with the difference appearing as early as week two.11
That's an intriguing result worth watching. It is not a reason to reach for creatine as mood support. It's a single small trial, in a specific clinical population, using creatine strictly as an add-on to prescribed medication and clinical care — a research signal, not a self-care instruction. A D means "insufficient human evidence to establish the benefit," and that's exactly where this sits. If you're dealing with depression, that's a conversation for a clinician, full stop.
Monohydrate vs. everything else
The one-line answer: buy creatine monohydrate. Nearly every study behind the grades above used it, it's inexpensive, it's about as absorbable as a supplement gets, and it's well tolerated.24 If you want maximum purity, look for Creapure®, a branded high-purity monohydrate; if you get drug-tested for sport, look for NSF Certified for Sport or Informed Sport on the label. That's the entire decision for most people.
There's one worthwhile tweak within monohydrate: a micronized version is milled into a finer powder so it disperses in water more easily and tends to be a little gentler on the stomach. It's the same molecule and the same evidence base — just better mixability, which is a real quality-of-life upgrade if you've ever choked down gritty creatine.
The other forms exist mostly to justify a higher price, and the marketing runs ahead of the data:
- Creatine HCl is sold on better solubility and a smaller dose. The solubility is real, but there's no head-to-head outcome evidence that it builds more strength or muscle than monohydrate, and the "less bloating" pitch leans on a water-weight fear that's overstated to begin with.2
- Buffered creatine (Kre-Alkalyn) claims to survive stomach acid better. A controlled comparison found it no more effective than monohydrate — and monohydrate isn't meaningfully broken down in the stomach anyway.2
- Creatine ethyl ester has actually underperformed monohydrate at raising muscle creatine in testing.23
- Liquid creatine ("serum") is unstable and degrades toward creatinine over time, which is not what you want to be swallowing.
- Gummies and chews are a delivery format, not a different molecule. They're fine if the label lists real monohydrate and the dose adds up — you're paying for convenience, which is a legitimate thing to buy, just know that's what it is.
None of the newer forms are scams, exactly. They're just a more expensive route to the same place, and none has cleared the bar of beating monohydrate where it counts. Until one does, "monohydrate is the evidence standard, everything else is marketing" is the accurate summary.
How to take it: dose, and why loading is optional
The dosage card above has the short version — 3–5 g a day, every day. Here's the nuance.
Loading is optional. The classic loading protocol is 20–25 g/day split into four or five smaller doses for five to seven days, then dropping to a 3–5 g maintenance dose. Loading saturates your muscles faster — roughly a week instead of about a month — but the endpoint is identical either way.5 The trade-off is a bigger early water-weight bump and a higher chance of stomach upset from the large doses. If you're not in a hurry, skip it: 3–5 g a day gets you to full saturation in three to four weeks with none of the downside.
Bigger athletes can sit at the top of the range. The 3–5 g maintenance dose covers most people, but a larger person carries more muscle — a bigger tank to keep full — so the top of the range (around 5 g, occasionally a touch more) makes sense if you're heavily built.1 There's no need to chase precision; the goal is simply to keep stores saturated, and a rounded daily scoop does that.
Timing barely matters. Because the benefit comes from saturated muscle stores rather than an acute pre-workout hit, the hour you take it is close to irrelevant — what matters is not skipping days, including rest days. With or without food is fine. Taking creatine with a carb- or protein-containing meal modestly improves how much your muscle retains, and food helps if your stomach is sensitive — but an empty stomach works too. And there's no need to cycle off; long-term daily use holds up in the safety literature.1
Safety, and the myths worth ignoring
For healthy adults, creatine has one of the strongest safety records of any supplement on the shelf. Across hundreds of controlled trials at 3–5 g/day, side-effect rates track placebo.12 Exactly one effect is genuinely documented: a small, temporary gain in water weight, held inside muscle — not fat, and not gut bloating. Almost everything else on the standard scare list is myth, misread lab value, or dose problem. Two large evidence reviews have worked through the popular fears one at a time and landed in the same place: the scary claims mostly aren't supported.23
The two that trip people up most:
- Kidneys. Creatine spontaneously converts to creatinine, so supplementing raises your serum creatinine — a normal metabolic byproduct, not a sign of damage. Because doctors estimate kidney function from serum creatinine, a creatine user can get a lab result that looks like reduced function when nothing is wrong. The fix is simple: tell your doctor you take creatine before any kidney panel. In healthy people at normal doses, meta-analyses show no harm to kidney function.
- Hair loss. The fear traces to a single 2009 study that measured a hormone, not a hair, and was never the slam dunk it's quoted as.

Who benefits most — and who won't notice much
Creatine is high-floor, not magic, and honesty about who it helps is more useful than blanket hype.
Biggest responders are people with low baseline muscle creatine — vegetarians, vegans, and low-meat eaters. They start furthest from saturation, so they have the most room to gain, and they tend to feel the difference most clearly.24 Anyone doing resistance, power, or sprint-style training gets a solid, well-documented benefit, and older adults have a specific case for it: preserving muscle and strength as they age.7
Who won't notice much:
- Non-responders. A meaningful minority of people — often heavy meat eaters or those with naturally high stores — are already close to saturation, so topping up does little.2
- Pure endurance athletes. The ergogenic signal is weaker for long-duration efforts, and the small water-weight gain can be an unwanted trade for some.
- People who don't train. Creatine amplifies a training stimulus. Take it and sit still and the strength-and-size payoff largely evaporates — though the brain and other tissues still use it, which is partly why the cognition research exists.
If you train and eat little meat, creatine is close to a no-brainer. If you're a saturated, endurance-focused athlete, temper your expectations — it may simply not do much for you, and that's a legitimate finding, not a dosing error.
