For most healthy adults, rhodiola is well tolerated, and the effects people actually report are mild: dizziness, headache, trouble sleeping, and either a dry mouth or more saliva than usual.1 That's the honest headline, and it's where the standard monograph stops.

Here's what it leaves out. That reputation rests almost entirely on trials lasting four to twelve weeks — the NIH's complementary-health center calls rhodiola "possibly safe for up to 12 weeks" and goes no further.1 It measurably slows the enzyme that clears warfarin and phenytoin.5 Published case reports link it to a manic episode, to a fast heart rhythm alongside an antidepressant, and to a serotonergic reaction on paroxetine.627 And a 2026 analysis of ten US-market products found several didn't contain what their labels claimed.12 Rhodiola looks mild as far as anyone has looked — and "as far as anyone has looked" is carrying a lot of weight in that sentence.

The side effects people actually report

When rhodiola causes problems in studies, they are minor and they show up in a short, consistent list. Four independent sources — the NIH's complementary-health center, Memorial Sloan Kettering, the Merck Manual, and the Department of Defense's supplement-safety program — name essentially the same handful of effects.1234 That agreement means this isn't a herb with a hidden common side effect nobody has noticed.

Feeling wired: insomnia, restlessness, jitteriness

This is the one the monographs bury and the one most likely to affect you. Rhodiola is stimulating rather than sedating, and insomnia appears in the NIH's own list of reported side effects.1 Where ashwagandha's classic complaint is drowsiness, rhodiola's is the opposite — people describe feeling revved, restless, or unable to switch off at night.

Timing is usually both the cause and the fix: rhodiola is a morning supplement, so if you take it after lunch and sleep badly, move it to breakfast before you conclude it doesn't agree with you — our guide to rhodiola dosage and timing goes into the detail. The DoD frames the reassuring safety record around amounts up to 600 mg daily,4 and several trials sat well under that — 340 mg a day in a small anxiety pilot, 364 mg a day in a randomized trial in shift-working nursing students.811 If a 500 mg capsule leaves you jittery, less is the obvious next experiment.

Dizziness and headache

Dizziness is the single most consistently reported effect across sources.1234 In a 10-person open-label pilot in generalized anxiety disorder, adverse events were generally mild or moderate, the most common being dizziness and dry mouth.8 Headache shows up in the NIH's and the DoD's lists.14 Both tend to be mild and to appear early.

Dry mouth — or the opposite

The oddly specific pairing you'll see everywhere: rhodiola is associated with either dry mouth or excess saliva production.13 The DoD spells it out the same way, listing "dry mouth, or (the opposite) excess saliva."4 Reports genuinely run in both directions, which is a fair signal that these are minor, idiosyncratic effects rather than a reliable pharmacological one.

What the trials reported as a whole

The tolerability record here is genuinely good, and this page shouldn't pretend otherwise. A systematic review of 11 placebo-controlled randomized trials concluded that "only few mild adverse events were reported."9 In a randomized, double-blind trial of 48 nursing students taking rhodiola or placebo for 42 days, the total number of adverse events did not differ between the two groups.11 Nothing in the trial literature suggests rhodiola is risky over a couple of months.

The caveat sits underneath that record rather than against it: the trials are small and the methodology is weak. A systematic review of rhodiola for physical and mental fatigue found that all 11 included studies carried either a high risk of bias or reporting flaws that made their validity impossible to assess.10 The DoD is blunter — rhodiola studies "suffer from poor methods and small sample sizes, so the strength of the evidence is very low."4 A clean safety record built on a thin evidence base is a weaker guarantee than it looks.

Drug interactions: the part most pages get thin

This is where a generic monograph leaves you without an answer. Rhodiola is not inert, and several of its interactions have a specific mechanism and a specific list of drugs attached.

Blood thinners and seizure medicine: the CYP2C9 finding

The most concrete human interaction data on rhodiola comes from a randomized crossover study in 13 healthy volunteers, who took a probe cocktail of drugs metabolized by five different liver enzymes with and without 14 days of pretreatment with a commercial rhodiola product. The result: a statistically significant 21% decrease in the losartan metabolic ratio, indicating reduced CYP2C9 activity, with no significant effect on the other four enzymes.5

In plain language: rhodiola slows one of the liver enzymes that clears certain drugs out of your bloodstream, so more of the drug stays in circulation than the prescribed dose assumes. For most medicines a 21% shift wouldn't matter. It matters for drugs with a narrow therapeutic index, where the gap between the right amount and too much is small — and the study's authors named the two that concern them: phenytoin and warfarin.5 Memorial Sloan Kettering flags the same pair, noting that rhodiola modestly inhibits CYP2C9 and may affect drugs metabolized by it, "particularly those with a narrow therapeutic index such as phenytoin and warfarin."2 Merck states directly that rhodiola may increase levels of warfarin.3

If you take either drug, this is a conversation with your prescriber or pharmacist before you start — not a reason to change a dose yourself. Never adjust prescription medication around a supplement.

Blood-pressure medication

The NIH notes that interactions between rhodiola and losartan, a blood-pressure drug, have been reported.1 Losartan is the same drug used as the CYP2C9 probe above, which is where much of that signal originates.5 Separately, both Memorial Sloan Kettering and Merck caution that rhodiola may affect blood pressure, so combining it with antihypertensive medication could push readings lower than intended.23

Antidepressants

Two threads converge here. The mechanism: Memorial Sloan Kettering notes rhodiola has monoamine oxidase inhibition activity and may increase serotonergic side effects, and Merck warns that serotonin syndrome may occur when it's combined with serotonergic medications.23 The cases: Memorial Sloan Kettering describes a 26-year-old woman who arrived at an emergency department with a fast, irregular heartbeat after three days of taking rhodiola alongside her antidepressant.2 Separately, a 2015 case report describes a 68-year-old woman on paroxetine who developed restlessness and trembling after adding rhodiola — which the authors read as a serotonergic syndrome.7

Scale that honestly. These are two published case reports, not a rate, and case reports cannot tell you how often something happens. But documented cases plus a plausible mechanism are where "ask your prescriber" is a real answer rather than a hedge. This isn't a claim that rhodiola is dangerous with SSRIs — it's a claim that the combination shouldn't be a solo decision.

Diabetes medication

Merck notes that rhodiola could cause hypoglycemia, particularly in people taking antihyperglycemic medication.3 If you manage blood sugar with medication, that's another item for the pharmacist conversation.

Stimulants

The practical version of "don't stack it with a pre-workout and three coffees." Rhodiola's own activating profile stacks with caffeine and other stimulants, and Memorial Sloan Kettering notes that its MAO inhibition activity may enhance the blood-pressure-raising effect of CNS stimulants — while describing that interaction's clinical relevance as yet to be determined.2 If you're already caffeinated to the eyeballs, rhodiola is the wrong addition.

Immunosuppressants

Brief but real: Merck states that rhodiola may counteract the effects of immunosuppressants.3 For anyone whose transplant or autoimmune treatment depends on those drugs working, that's a clear reason to check first.

Who should not take rhodiola

  • Pregnancy and breastfeeding — avoid. The reason differs from ashwagandha's, and the difference is worth stating. Ashwagandha carries a pregnancy contraindication based on a specific documented concern, detailed in our ashwagandha side effects guide. Rhodiola's is an absence of data: the NIH's position is that "little is known" about whether it's safe in pregnancy or while breastfeeding,1 and the DoD advises avoiding it if you're pregnant or nursing until more research exists.4 Nobody has established a harm; nobody has established safety either. In pregnancy, unstudied is reason enough to skip it.
  • Anyone with bipolar disorder or a history of mania. A 2022 case report describes a manic episode associated with rhodiola,6 and Merck notes a case of a manic episode precipitated in a patient taking it as an exercise supplement.3 Case reports, not a rate — but rhodiola's activating profile makes the concern plausible, and this is a group with a lot to lose from a wrong guess.
  • Anyone taking warfarin, phenytoin, an antidepressant, blood-pressure or diabetes medication, or an immunosuppressant. Pharmacist first, for the reasons above.
  • Anyone with an autoimmune condition — carefully. The reasoning is indirect: rhodiola is marketed for immune support, and Merck's caution about it counteracting immunosuppressants points the same way.3 The evidence is thin and we won't inflate it, but run it past the clinician who manages the diagnosis.
  • Children. Not studied. The DoD advises against giving it to a child until safety research exists.4

The 12-week ceiling

Every page that tells you rhodiola is safe is quoting short trials. The NIH's wording is precise and deliberate: rhodiola is "possibly safe for up to 12 weeks."1 The DoD says the same thing and names the boundary explicitly — safe in studies up to 12 weeks at amounts up to 600 mg, "but the safety of long-term use is unknown."4

That is the edge of the map. Taking rhodiola daily for a year isn't known to be harmful; it also isn't known to be safe, because no study has run long enough to find out. Those are different statements, and most articles collapse them into "rhodiola is safe long term." The popular workaround — cycling, a few weeks on and a few weeks off — is convention rather than evidence: no trial has tested whether it prevents anything, because there's no documented long-term harm to prevent yet.

The NIH's overall verdict is that "there isn't enough reliable evidence to determine whether rhodiola or its components are useful for any health-related purpose."1 That cuts both ways — it's the reason to keep expectations modest about the benefits, and the reason a clean side-effect list should be read as understudied rather than proven harmless. When "rhodiola gave me side effects" is really a product problem

Two of the most common complaints about rhodiola — "it did nothing" and "it made me jittery" — can both come from the same source: the capsule doesn't contain what the label says.

Only Rhodiola rosea contains meaningful amounts of rosavins. Rhodiola crenulata, the documented substitute, contains salidroside but not rosavin, which is why unexpectedly low rosavin content is treated as a marker of species substitution.1213 The Botanical Adulterants Prevention Program's bulletin catalogues how routine this is: in one analysis of 39 UK products labeled R. rosea, seven unregistered food supplements — about 25% — turned out to be other Rhodiola species, one adulterated with synthetic 5-HTP; in a separate screening of samples from China and the UK, eight of 11 samples labeled R. rosea (72.7%) contained other species.13

The US market has now been sampled too. A 2026 analysis in PLoS One tested 10 commercial products bought in the US — seven capsules and three tinctures — and found rosavin content ranging from 0.01% to 3.08%, with what the authors call "substantial aberrations from advertised biomarker amounts."12 Two capsules measured well below their advertised rosavins: one labeled 3.0% came in at 1.51%, and one labeled 1.0% at 0.19% — nearly five times short, with its salidroside similarly low.12 A third contained just 0.01% rosavins, which the authors read as an unexpectedly low R. rosea extract content.12 One product appeared to contain undisclosed synthetic salidroside.12 Pesticides were below detection limits, but all seven capsules contained trace arsenic, cobalt and lead, two of them with notably elevated cobalt and arsenic that the authors said would need follow-up speciation testing to interpret against federal minimal-risk levels.12

Ten products is a small sample, and we're not stretching it into "most rhodiola is fake." What it establishes is that variation between bottles is real and large. Why that happens is no mystery: most raw material is wild-harvested, the plant takes years to regenerate, and resource depletion has made authentic material scarcer and dearer — conditions that reward substitution all the way down a long supply chain.13

What to look for on a label: the full binomial Rhodiola rosea rather than just "rhodiola"; standardization to both rosavins and salidroside (3% / 1% is the standardized extract spec suppliers sell to manufacturers13); and third-party testing you can verify. For reference, the US Pharmacopeia standard for the raw material is not less than 0.3% rosavins and not less than 0.08% salidroside.13

Rhodiola versus ashwagandha on safety

The instinctive read — rhodiola is the gentler adaptogen — is probably right, and right for a less flattering reason than people assume.

Ashwagandha has both the stronger efficacy evidence and the more serious documented harms: a rare but real liver-injury signal, and a firm pregnancy contraindication. Rhodiola has neither the trial base nor the case literature. Its two serious case reports and its short, small trials are a thinner record in both directions. Some of rhodiola's gentle reputation is genuine tolerability; some of it is simply that far fewer people have looked. Absence of evidence is not evidence of safety.

If rhodiola doesn't agree with you

Most reported effects are mild and resolve when you stop. Before you write the herb off entirely, three things are worth ruling out, in this order:

  1. Timing. If you're sleeping badly or feel wired in the evening, move the dose to the morning. This resolves the single most common complaint.
  2. Dose. The studied range runs roughly 200–600 mg a day, and several trials sat near its lower end — 340 mg in the anxiety pilot, 364 mg in the nursing-student trial.811 If you started on a 500 mg or 1,000 mg capsule, less may simply suit you better; the dosage and timing guide has the full breakdown.

If none of that helps, stopping is a perfectly reasonable answer. You do not need a randomized trial's permission to notice that something makes you feel worse.

When to stop and contact a clinician

Some reactions aren't in the "adjust the timing" category. Stop rhodiola and get medical advice if you experience a fast or irregular heartbeat, chest discomfort, fainting or near-fainting, or a marked and unusual change in mood, sleep or energy — particularly if you take an antidepressant or have a history of mania.267 Don't troubleshoot symptoms like these on your own, and don't change any prescription medication without your prescriber.

The bottom line

For most healthy adults, rhodiola is one of the better-tolerated supplements in the adaptogen aisle, and the effects people report — dizziness, headache, dry mouth or extra saliva, and trouble sleeping on a late dose — are mild and usually fixed by taking it earlier or taking less. The list that needs a pharmacist before you start is short and specific: warfarin, phenytoin, blood-pressure medication, antidepressants, diabetes medication, immunosuppressants. Pregnancy and breastfeeding are a skip, not a maybe. And the reassuring safety record has a hard boundary at about 12 weeks, because that's where the research stops.
